Registration at Jefferson Middle School
Registration for families new to the Jefferson attendance area will be held from 1:00 - 3:00 daily at the Jefferson campus. Please see below for a list of items to bring with you when registering.

What items should I bring when I register my child(ren)?
- Proof of Residence
1 of the 2 items below - NOTHING ELSE CAN BE ACCEPTED!
- mortgage or lease agreement (within the last 30 days) OR
- electric or water bill
- Driver's License, Texas ID or Military ID
- Withdrawal Form - if enrolling during the school year
- Report Card
- Birth Certificate - official copy
- Social Security Card
- Immunization Records
- Standardized Test Scores (TAKS) - or any other Testing Data available
- ARD/IEP copies (most recent) - only if child receives spec. ed. services
- Divorce and/or Legal Guardianship situations - Details (see #10)
What forms might I be able to print or fill out ahead of time and bring when I register my child(ren)? (not all forms may be applicable to all family situations)
